CLE Credit Forms

CLE CREDIT REQUEST FORM - for attendees and speakers

Attendees & Speakers should complete a CLE Credit Request Form. Print your name and email, indicate the credits you are seeking and check the sessions you were present for on the form.

Speakers should check the speaker box and fill in the session you taught.

Everyone should return the CLE Credit Request Form to the ALFN Registration Desk in the Ballroom Foyer area prior to leaving the conference.

CLE SIGN-IN SHEETS - for attendees and speakers

All attendees and speakers must sign the CLE Sign-In Sheet and mark their arrival time to confirm their attendance for CLE credit. Attendees and speakers must personally sign in—no one can sign in for another person. If attendees and speakers do not sign in, they will not receive CLE credit even if they complete a CLE Credit Request Form.

KANSAS CLE AFFIDAVITS - for attendees who want Kansas CLE credit

There are 2 Kansas CLE Affidavit forms. Complete both forms to receive Kansas CLE credit.

If a Kansas attorney plans to attend General Session 3 and/or Breakout Session 7, they should complete the affidavit that lists 2.00 Law Practice Management credits.

If a Kansas attorney does not plan to attend General Session 3 and/or Breakout Session 7, they should complete the other affidavit.

Attorneys should return the affidavits to the ALFN registration desk prior to leaving the conference, along with the CLE Credit Request Form. After NBI has verified attendance and submitted attorney credits online to Kansas CLE, they will send each attorney a copy of the affidavit for their records.
